This scheme provides financial assistance to the following 4 categories of patients. The main objective

of the scheme is to provide care and protection to the weaker section of our community. Those who are availing any other financial assistance under any other Govt. scheme are also eligible for this scheme.

SAMASHWASAM SCHEME - I

This scheme is meant for kidney patients belonging to BPL families who are undergoing dialysis at least once in a month. The assistance is Rs.1100/- per month.

ELIGIBILITY CRITERIA

1.     Patient should be from a BPL (Below Poverty Line) family.

2.     Applicant must be a nationalized bank account holder.

DOCUMENTS TO BE SUBMITTED

1.  Attested copy of BPL certificate / BPL ration card.

2. Medical certificate issued by reputed nephrologist from Govt / Private hospital where the patient is undergoing monthly dialysis. The medical certificate should include the date from which the patient started the dialysis treatment.

3. Life certificate and latest medical certificates to be submitted in every 6 months to the Executive Director, Kerala Social Security Mission to continue the scheme benefits.

4. The applicant must be an account holder in a nationalized bank. The copy of the bank pass book must also be submitted.

4. Attested copy of Aadhaar card / confirmation slip received after Aadhaar card registration.

HOW TO APPLY

Application form available in Vayomithram Office at Municipalities / ICDS project office / Panchayat offices / Corporation/ Municipal offices/ Anganwadi workers / KSSM website / KSSM office. Application form duly filled with supporting documents to be submitted to ICDS Project Officers. ICDS officer will verify and submit application to the Executive Director, Kerala Social Security Mission.

SAMASHWASAM SCHEME – II

This scheme is meant for the Kidney/liver transplanted patient whose annual family income is below Rs. 1 lakh. The rate of assistance is Rs. 1000/- per month and limited to 5 years from the date of transplantation.

ELIGIBILITY CRITERIA

       Annual family income should be less than 1 lakh.

DOCUMENTS TO BE SUBMITTED

1.      Medical certificate issued by the concerned medical officer from Government / Private hospital certifying that the patient has undergone liver/kidney transplantation surgery and the patient is still under treatment.

2.   Copy of family annual income certificate issued by Village Officer.

3.   Attested copy of hospital discharge sheet given after liver transplantation surgery.

4.      Copy of the applicants bank pass book.

5.      Life certificate and latest medical certificates to be submitted in every 6 months to Executive Director, Kerala Social Security Mission to continue the scheme benefits.

6.      Attested copy of Aadhaar card / confirmation slip received after Aadhaar card registration.

 

HOW TO APPLY

Application form available in Vayomithram Office at Municipalities / ICDS project office / Panchayat offices / Corporations / Municipal offices/ With Anganwadi Workers / KSSM website / Kerala Social Security Mission office. Application form duly filled and supporting documents to be submitted to ICDS Project Officers. ICDS officer will verify and submit application to the Executive Director, Kerala Social Security Mission.

SAMASHWASAM SCHEME – III

Under this scheme financial assistance of Rs.1000 per month is given to the patients who are affected Hemophilia due to the lack of blood clotting factors 8,9,11,13 and related diseases.

 ELIGIBILITY CRITERIA

1.      There is no income ceiling limit. 

DOCUMENTS TO BE SUBMITTED

1.      Certificate issued by doctor in Medicine / Pediatric / Hematology department with counter signature of Department Head / Hospital Superintendent certifying that the beneficiary is hemophilia patient.

2.      Applicant must be an account holder in a nationalized bank. The copy of the bank pass book also be submitted.

3.      Life certificate and latest medical certificates to be submitted in every 6 months to Executive Director, Kerala Social Security Mission to continue the scheme benefits.

4.      Attested copy of Aadhaar card / Any ID card with photo / confirmation slip received after Aadhaar card registration.

SAMASHWASAM SCHEME – IV

Under this scheme financial assistance of Rs.2000/- per month is distributed to the people other than Tribal suffering from Sickle Cell Anemia.

ELIGIBILITY CRITERIA

1.      Family annual income should be less than 1 lakh.

2.      Beneficiary should not fall under ST category.

3.      Sickle Cell Trait (HBAS) patients are not eligible.

DOCUMENTS TO BE SUBMITTED

1.      Medical certificate issued by a reputed pathologist from Kozhikode Medical College/ Government hospital stating that the patient has HBSS / HBS Combination infection. Certificate should be in the prescribed format.

2.      Community certificate issued by the Village Officer.

3.      Attested copy of BPL ration card / Income certificate issued by the Village Officer.

4.      Attested copy of Aadhaar card / Any ID card with photo / confirmation slip received after Aadhaar card registration.

5.      The applicant must be an account holder in a nationalized bank. The copy of the bank pass book also should be submitted.

6.      Life certificate and the latest medical certificates to be submitted in every 6 months to Executive Director, Kerala Social Security Mission to continue the scheme benefits.
